    • A variable rate defibrillator charging circuit includes a power transformer that accepts an internal battery output and the output from an AC-to-DC converter on separate primary windings. The secondary winding of the transformer is coupled through a rectifier to a storage capacitor. The transformer operates in a fly-back mode whereby energy is discharged into the storage capacitor when the power supplies discontinue charging the transformer. After energy in the transformer is discharged into the storage capacitor, the power supplies are reactivated, recharging the transformer. The separate primary windings of the transformer provide a fast, reliable, low cost means for charging the storage capacitor with multiple power supplies. The AC power source feeding the AC-to-DC converter is detachable allowing the defibrillator to be operated remotely whereby the storage capacitor is charged solely from the internal battery. The charging circuit monitors the output voltage levels of both the battery and the AC-to-DC converter and enables or disables the power supplies in accordance to their ability to charge the storage capacitor within a predetermined amount of time. The energy in the storage capacitor, which is eventually discharged into a patient, is monitored to prevent overcharging.

    • An automatic fibrillator for inclusion in an implantable defibrillator. Fibrillation induction takes place while the patient's heart is beating at a known rate, and the fibrillation inducing pulse is delivered at a time based on the expected or measured natural refractory period of the patient's heart, at the heart rate in effect during fibrillation induction. In its preferred embodiment, fibrillation induction takes place during overdrive pacing, and the timing of the pulse takes place using an overdrive pacing pulse as a time reference. The period between the pacing pulse and the fibrillation inducing pulse may be determined as a function of actual measurement of the patient's refractory period, either directory, or by means of measurement of the Q-T interval of the patient's heart.

    • A cardiac stimulator apparatus for effecting cardioversion or defibrillation in the event of pathologic tachycardia or ventricular fibrillation in which the rate of heart depolarization signals is compared to a predetermined heart rate value indicative of the onset of tachycardia while, simultaneously, the heart's pre-ejection period is monitored to sense whether an increase in heart rate above the predetermined value is accompanied by a decrease in the pre-ejection period. If not, a pathologic rather than a physiologic episode is diagnosed and a cardioversion pulse protocol is initiated. By also monitoring the mechanical pulse of the heart, if an inordinately high heart rate is accompanied by a low or non-existing mechanical pulse, ventricular fibrillation is diagnosed and the cardioverter is triggered to shock the heart back into sinus rhythm. If the mechanical pulse rate tracks the electrical rate of depolarization, tachyarrythmia is confirmed. The diagnosis may be further confirmed by taking into account the rate at which the heart rate increases.
