    • A data carrier provides health related information regarding a patient, and facilitates compliance with HIPAA by the obscuring the association between the name of the patient and the health related information when the data carrier is to be discarded. The data carrier includes a release liner, a health information label having an upper surface and a lower surface, the label including a first area on the upper surface for indicia specifying health related information and a second area on the upper surface for indicia specifying the identity of a patient, and a pressure sensitive adhesive coating on the lower surface of the health information label, securing the health information label to the release liner. A masking label is either integral with the health information label, or the liner. By various arrangements the masking label is either removed from the health information label, thereby disassociating the patient identity and the health related information, or the masking label is secured to the health information label so as to cover and obscure the name of the patient or the information, or both.

    • A business form includes a paper carrier sheet having an upper surface and a lower surface, a barrier layer on an area of the upper surface of the paper carrier sheet, a silicone coating extending over the barrier layer, and a label assembly. The label assembly includes a label ply and a pressure sensitive adhesive coating. The label assembly is secured to the silicone coating and to the upper surface of the carrier sheet by the pressure sensitive adhesive. The label ply has a die cut through it that defines a label portion and a lip portion. The label portion is secured only to the silicone coating, and the lip portion is secured to both the silicone coating and to the upper surface of the carrier sheet. When the business form is printed with a printer, such as a laser printer, the form moves through the printer in an orientation in which the lip portion leads the label assembly. This results in the label portion being less likely to be removed from the carrier sheet inadvertently.

    • An optically decodable security document is provided comprising a security image area defined on a face of the document. The security image area is divided into a plurality of image element cells defining an array of image element cells across the security image area. The image element cells define first and second diagonal axes and a cell center positioned at the intersection of the first and second diagonal axes. A first type of printed tiling element is printed in selected ones of the image element cells to define a printed security image within the security image area.     • A security document is prepared by designating specialized tiling elements to be printed in selected image element cells across the face of the document. In accordance with one embodiment of the present invention, a method of preparing a security document is provided comprising the steps of: (i) designating a security image area on a face of a document; (ii) dividing the security image area into a plurality of image element cells, such that the image element cells define an array of image element cells across the security image area; and (iii) designating respective printed tiling elements to be printed in selected ones of the image element cells to define a printed security image and a printed complementary security image within the security image area. Individual ones of the respective printed tiling elements comprise printed units positioned within the respective image element cells. The printed units are positioned such that image element cells including the respective printed tiling elements define substantially identical repeating printed elements, and such that individual ones of the repeating printed elements comprise apportioned repeating printed elements. The apportioned repeating printed elements are defined by a combination of printed units from neighboring image element cells.

    • A construction for a laminated card or label is provided including a transparent film having a pressure sensitive adhesive on one surface thereof and liner stock having first and second major surfaces. The first surface has a release coating adhered to the pressure sensitive adhesive on the film. Variable information may be printed on the second surface of the liner stock within the die cut area, and the die cut card removed from the transparent film and replaced printed side down onto the adhesive surface of the film, thereby laminating the card to the film. The card is then peeled away from remainder of the construction at the die cut around the film and folded with the indicia facing outward to laminate the sides the card together. The printed surfaces of the card are direct contact with the adhesive, thus preventing tampering of the card.
